Abstract I present the case of old age man, who presented with pain right side abdomen huge anterior abdominal wall swelling, along loose motion in one hospitals an Asian country. His inflammatory markers were unremarkable, and his ultrasound showed fluid-filled bowel loops, no ascitic fluid. The appendix could not be visualised. On examination, he had tender distended sounds exaggerated. Laparotomy was performed it came out to missed appendicitis which buried into caused pus track down anteriorly appear as abscess its rare complications. Appendectomy done alongside exploration. Pus drained. patient improved went home second post-operative week.
